The forms used by the plant protection section in biological activities will be known by the following title and designations:

(1)           Certificates of apiary inspection of the Plant Industry Division:

(a)           Certificate of Apiary Inspection‑‑BS‑1;

(b)           Uniform Certificate of Apiary Inspection ‑‑BS‑4;

(2)           Apiary Registration Form‑‑BS‑2;

(3)           Honeybee Registration List‑‑BS‑3;

(4)           Application for Scientific Permit‑‑BS‑5;

(5)           Scientific Permit‑‑BS‑6;

(6)           Permit to Move Bees or Apiary Equipment ‑‑BS‑7;

(7)           Compliance Agreement‑‑BS‑8;

(8)           Fumigation Permit and Application Form‑‑BS‑9;

(9)           Bee Equipment Fumigation Work Requisition‑‑BS‑10;
