The following forms are used:

(1)           "NCD‑3 Agreement Covering USDA Donated Commodities" is used by institutions who qualify to feed inmates and patients and by schools for the school lunch program;

(2)           "NCD‑3 Summer Camps Agreement" is used by agencies operating a nonprofit feeding service in a summer camp for children at high school grade and under when only USDA donated foods are used; and

(3)           "NCD‑8 Application for USDA Donated Foods" is used by agencies requesting USDA donated foods.
