18 NCAC 12 .0426             ADDITIONAL INFORMATION FOR FEE WAIVER

If a nonprofit principal requesting fee waiver was formed within 12 months of the request for a waiver and has no net assets or fund balance information, the fee waiver request shall include the following information:

(1)           A statement signed pursuant to Rule .0418 of this Chapter verifying that the nonprofit has no more than two lobbyists;

(2)           A statement signed pursuant to Rule .0418 of this Chapter verifying the nonprofit's formation date; and

(3)           A statement signed pursuant to Rule .0418 of this Chapter verifying that the nonprofit has no fund balance information or net assets.
