SECTION .0300 - RENEWAL OF LICENSE

21 NCAC 03 .0301             RENEWAL REQUEST FORM

Requests for license renewal shall be submitted on the form provided by the Board. The renewal form shall contain the following:

(1)           The licensee's full name;

(2)           The licensee's address;

(3)           The licensee's job concentration area, address, and job title;

(4)           Verification of a current Board of Certification, Inc. certification via current card; and

(5)           Affirmation the licensee has not been convicted of any violations of law in the last 12 months.
